Discussing Questions for Neuros

Hi all,

Happy new year! I am going to post these questions to MedHelp for their Neuro's to answer, and thought since many of you check that board we could get a discussion started on these questions here, although we may not really know the answers since we are not Neuros!

I have read through alot of info on our twitches, but have not found a real answer to what is to me a fairly obvious question. What is an ALS fascilation like? We'll use an example. Say someone has a twitch in their hand, and it is because of ALS attacking the muscles of the hand. What will this twitch be like, will it be constant? Will it come and go? Will it be bad Monday, better Tues., terrible Wed., etc. etc.? How often would you twitch?

With the same example, if you had ALS starting in your hand, with its fairly small muscles, how soon would you see weakness, or in your hand would you pretty much always present with weakness first, in turn, leaving ALS cases where people don't have immediate muscles weakness to large muscle groups where ALS started?

By the time you develop cramping in muscles that have ALS, is that a step beyond fascs and woudl certainly have muscle weakness associated with that?

Is an EMG always neccessary, or can ALS be distinguished from BFS by a good neuro with a thourogh neurological exam?

How long do you have to have twitches with no muscle weakness, atrophy, or other associated ALS symptoms, before you can have a pretty good assurance that you don't have ALS, which is constantly progressing?

Well, this is what I will be posting to MedHelp Neuro forum, I think these are valid questions, and hopefully we can all benefit from them, and please, everyone throw their thoughts on these here as well.
