With me it started together with ME and the muscle fatigue was so great at a certain point - more or less 3-4 years after onset of the twitching - that for some months I could hardly walk. But now there is no noticeable muscle fatigue at all and my muscles are very strong, but still twitch.

Thanks for the reply. Don't get me wrong, the problem was severe for 3 or 4 years, in conjunction with chronic fatigue; it receded afterwards, though never completely, and it only recurred recently, at the same time as the tremor, which was new, in the context of a major upheaval in my life...
